      For School Year

     
     2017/2018

     
     3 and 4 years of age (Head Start and Pre-K)
    • DTaP - 4 doses
    • Polio - 3 doses
    • MMR - 1 dose - must be received on or after the 1st birthday**
    • Hib - 3 doses (one of which must be on or after 12 months) OR 1 dose after 15 months through the age of 4 years; no Hib required from 5th birthday on
    • Hepatitis B - 3 doses
    • Varicella - 1 dose, on or after the 1st birthday** (if the child has not had chickenpox)
    • PCV (Pneumococcal conjugate vaccine) - 4 Doses (Children 24 months through 59 months meet the requirement if they have at least 3 doses with 1 dose on or after 12 months of age,** OR 2 doses with both doses on or after 12 months of age,** OR 1 dose, on or after 24 months of age.)** Otherwise, one additional dose is required. Children 60 months of age and older are not required to receive PVC vaccine.
    • Hepatitis A - 2 doses, first dose received on or after 1st birthday** and 2nd dose no sooner than 6 months after 1st dose
     
    Kindergarten - 6th Grade
    • DTaP - 5 doses, including one received on or after the 4th birthday** (unless 4th dose was received on or after the 4th birthday**, then 4 doses meets requirement). For students aged 7 years & older, 3 doses meet the requirement if 1 dose was received on or after the 4th birthday.
    • Polio - 4 doses, including one received on or after the 4th birthday** (unless 3rd dose was received on or after 4th birthday**, then 3 doses meets requirement)
    • MMR - 2 doses,*** first dose must be received on or after the 1st birthday** and at least 28 days apart
    • Hepatitis B - 3 doses
    • Varicella - 2 doses, if the child has not had chickenpox. The first dose must be received on or after the 1st birthday.**
    • Hepatitis A - 2 doses, first dose received on or after 1st birthday** and 2nd dose no sooner than 6 months after the 1st dose.
     
     
     7th Grade
    • DTaP - 3 doses, including one on or after 4th birthday** AND Tdap/Td* booster, 1 dose if at least 5 years have passed since the last dose of tetanus-containing vaccine 
    • Polio - 4 doses, including one received on or after the 4th birthday** (unless 3rd dose was on or after 4th birthday**, then 3 doses meets requirement)
    • MMR - 2 doses,*** first dose must be received on or after 1st birthday** and at least 28 days apart
    • Hepatitis B - 3 doses
    • Varicella - 2 doses, if the child has not had chickenpox. The first dose must be received on or after the 1st birthday**
    • Meningococcal (MCV4) - 1 dose; 1 dose of MCV4 is required on or after the student's 11th birthday. Note: If a student received the vaccine at 10 years of age, this will satisfy the requirement.
    • Hepatitis A - 2 doses, first dose received on or after 1st birthday** and 2nd dose no sooner than 6 months after the 1st dose.
     
    8th Grade
    • DTaP - 3 doses, including one on or after 4th birthday** AND Tdap/Td* booster - 1 dose within last 10 years 
    • Polio - 4 doses, including one received on or after the 4th birthday**(unless 3rd dose was on or after the 4th birthday**, then 3 doses meets requirement)
    • MMR - 2 doses,*** first dose must be received on or after the 1st birthday** and at least 28 days apart
    • Hepatitis B - 3 doses
    • Varicella - 2 doses, if the child has not had chickenpox. The first dose must be received on or after the 1st birthday**

    * Td is acceptable in place of Tdap IF a medical contraindication to pertussis exists
     
    ** Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry requirement.
     
    *** For K - 12 grade, Students vaccinated prior to 2009 with 2 doses of a measles-containing vaccine, and one dose each of rubella and mumps vaccine satisfy this requirement.
