Sports Physicals
Community Healthcare Center is having a FREE Sports Physical Clinic on Thursday, July 18th from 2pm to 5pm.
This event is open to all Middle School and High School Students, including those that participate in Youth Sports Leagues. All public school districts are welcome, including private schools and youth sports leagues. Don’t forget to bring that completed sports physical form that is required by your school district or youth sports league signed by a parent or guardian. There is a new U.I.L. sports physical form this year. So, please make sure your student's sports physical form has the year 2024 in the top right of the front page. Plus, if your child wears contacts or glasses, be sure to have those available at the event to pass the eye exam portion of the physical.
The FREE Sports Physical Clinic will be held at Community Healthcare Center located at 200 MLK Jr. Blvd., Wichita Falls, Texas 76301.
The student does not have to be a patient of Community Healthcare Center to receive a FREE sports physical at this event.
