Medicine Administration in School
Prescription Medications
- Prescription medications must be a prescription bottle from the pharmacy properly labeled with the child’s name, physician, name of the medication and direction for time and dosage. The pharmacy can supply two (2) labeled bottles for this purpose. Medications sent in baggies or unlabeled containers will not be given.
Medication will not be accepted if the label has been altered by hand.
A completed request signed by a parent/legal guardian and by the ordering physician/licensed prescriber must be presented with the properly labeled medication prior to administration.

Medications given at school must be brought to the school by the parent/guardian or an adult designee (18 years of age or older and not a current WFISD student).
Medications are stored in the nurse’s office in a locked cabinet and are supervised at all times.
Non-prescription (OTC) Medications
- OTC medications must be in the manufacture’s packaging, unopened, labeled with the student’s name and not expired.
- The WFISD Non-Prescription (OTC) Medication Authorization Form must be completed and signed by the parent/guardian and presented with the OTC medication.

- All OTC medications must be provided by and brought to the school by the parent/guardian or an adult designee (18 years of age or older and not a current WFISD student).
- Students are not allowed to carry OTC medications.
- The directions on the OTC packaging regarding age, dose and frequency will be strictly adhered to. A parent’s request to change the dosage or frequency of the OTC medication requires a physician’s written note stating the dosage and frequency of the medication to be given (fax will be accepted).
