Senate Bill 12

New legislation from the 89th Texas Legislative Session requires parents and guardians to provide annual consent for certain school based services. 

Parents must give consent before schools can provide health services.

Routine Health-Related Services Provided at Your Child’s Campus 

  • First aid and injury evaluation
  • Nursing Assessments 
  • Administration of medications upon written request and physicians orders. 
  • Monitoring of chronic health conditions (e.g., asthma, diabetes)
  • Routine health screening- Vision, hearing, and scoliosis screenings
  • Communicable disease or condition symptoms evaluation, exclusion, and readmission. 
  • Support during illness or physical symptoms at school
  • Health counseling and education (nutrition, hygiene, disease prevention, general wellness) 
  • Coordination of health services
  • Special procedures (upon written request and with physicians orders)
  • Heat illness prevention and injury support for student athletes

Any additional health-related services that the campus may believe are necessary and appropriate will necessitate separate written consent and parental involvement, unless during an emergency or as allowed by law.

How to complete the consent forms online

ParentSquare
1. Log in to your ParentSquare account.
2. On your post feed or search bar, look or type in "Annual Consent Form for Routine Student Health Services" 
3. Click on the link
4. Review each section
5. Indicate whether or not you consent 
6. Sign and submit

Skyward
1. Log in to your Skyward Family Access account.
2. On the left-hand side of the screen, click on "Online Forms"
3. Locate the New Parent Consent Forms.
4. Review each section
5. Indicate whether or not you consent
